Repeat Prescriptions
Repeat prescriptions can be obtained by any of the following procedures:
Order at the reception desk or leave in the box at reception (paper and pens are provided).
Send your request in by post for collection or return in an SAE.
Phone 631751 between 10.00am - 4.00pm Monday to Friday.
Request your prescription via the form below.
Whichever is your preferred method of ordering your repeat prescription please be sure to write or speak clearly and legibly.
Give your computer number from your computer record slip, give your full name and date of birth and state clearly all the items that you want repeating.Prescriptions are normally ready for collection after 2.00pm, 48 hours after ordering.
Please Note:
Please do not request medication that is not on repeat prescription.
To avoid delay and to help us with the processing please provide as much information as possible on the form.
